Application Requirements
Immunization Requirements
A strict immunization policy is enforced at UoMSOM. Compliance with immunization documentation for VSLO students mirrors UoMSOM students. All VSLO students must complete and submit AAMC Standardized Immunization Form prior to being approved to participate in any elective. Students who fail to document the following requirements will be delayed:

Tetanus/Diphtheria - Need Tdap within 10 years or Td within 10 years if patient has had one Tdap as an adult.

Measles - Given on or after the first birthday or evidence of immunity (antibody titer). Immunization dates must include month/day/year.

Rubella - Given on or after the first birthday or evidence of immunity (antibody titer). Immunization dates must include month/day/year.

Mumps - Given on or after the first birthday or evidence of immunity (antibody titer). Immunization dates must include month/day/year.

Varicella (Chickenpox) - Proof of two immunizations or evidence of immunity (antibody titer). Immunization dates must include month/day/year.

Tuberculin skin test (TST) - Two TB skin tests administered and interpreted within 12 months of the requested rotation. The most recent TST should be administered within 3 months of the rotation. Students can also obtain the IGRA blood test (QuantiFERON TB Gold/TSPOT), which must be within 12 months of the rotation and at three months of the same rotation they must fill out a University of Maryland Baltimore (UMB) Student Health TB Questionnaire. If the TB skin test or the IGRA is positive, documentation of a chest X-ray report and documentation of prophylaxis therapy, if completed is required. If the visiting student has a history of a positive TST or IGRA, documentation of a chest X-ray report within 5 years and a UMB Student Health TB Questionnaire is required. Within three month of the same rotation another UMB Student Health TB Questionnaire is required.

Hepatitis B - Hepatitis B vaccine series (three doses required) and a positive antibody titer. If the Hepatitis B titer is negative, please start the series again and obtain another Hepatitis B titer 4-6 week after the third dose.
- Influenza Vaccine - Seasonal influenza vaccination during the current season of the rotation.
Liability/Malpractice Insurance Requirement
Students must have a minimum of $1,000,000/$3,000,000 liability/malpractice insurance coverage. Documentation with dollar amounts and expiration date is required.
Health Insurance Requirement
Each VSLO student must upload copy of their current personal health insurance card to the supplemental document section of their application.
