Care and Support

The JI Linkage to Care Team supports people living with HIV in accessing needed care. Our trained patient navigators and nurse care coordinators provide help with:

• Identifying an HIV doctor or clinic
• Transportation to medical appointments
• Help scheduling appointments
• Establishing primary care, mental health care, or addiction services as needed
• Assistance with applying for housing, insurance, and other resources
• Support at your first medical visit
• Home visits

How We Can Help

We specialize in working with individuals who have a new HIV diagnosis, or are experiencing challenges staying in healthcare for any reason at all. Our navigators and nurses are passionate about partnering with people living with HIV to help each individual get on the path to good mental and physical health by providing HIV education, psychosocial support, and help overcoming healthcare system barriers.
