COVID-19 In-Person Screen for Outpatient Visit
All Clinic Staff:
1. Ask if Patient/Visitor has a fever, cough or shortness of breath.
If yes, ask the patient to put on a mask and sanitize hands.
2. Ask if they have traveled from China, Hong Kong, Iran, Italy, Japan or South Korea OR
Had close contact with anyone diagnosed with the new coronavirus (COVID-19), within 14 days of developing symptoms in #1.
If Yes to #1 AND #2
Ask Patient/Visitor to wear a facemask and inform them of airborne precaution policy. Say:
“Your answers mean we need to take some extra precautions. Would you please put on this mask, use this hand sanitizer, and go to that room? Please shut the door behind you. One of our team will be with you in a few minutes.”
Contact clinical supervisor and direct patient/visitor to a private closed room. Place a Contact and Airborne Precaution sign on room. No one should enter room without respirator, gown, gloves and eye protection.

If Yes Only to #1 (symptoms only)
Ask the patient to wear a mask, use hand sanitizer, and treat according to clinical judgement/guidelines for respiratory illness.
If Yes to Only #2 (travel exposure only)
Give the patient the CDC COVID-19 Information Sheet.
