Wraparound Fidelity Assessment System Evaluations
The Wraparound Fidelity Assessment System has been monitoring fidelity to the Wraparound Service Model (WSM) since 2006. This evaluation provides the state with extensive fidelity monitoring and child level outcomes data for youth involved in Wraparound programs. The data in these reports is provided through work with Wraparound Care Management Entities across four jurisdictions in the state. Data describing fidelity to the Wraparound model and youth/family satisfaction with services are collected through telephone interviews and in-person observations of child and family teams. Bi-annual reports are created and distributed to local and state agencies involved in either providing or funding Wraparound services in an effort to improved the quality and effectiveness of the Wraparound service delivery model.

Materials
For ALL youth referred to the CME we need a variety of documentation each month. We ask that this paperwork be sent to us within one week of the first Child and Family Team meeting:
- a completed WFAS referral form with your ETO or TCM youth/family ID written near the youth's name,
- a signed service consent/agreement (see the correct version below), and
- a court custody document for all youth who have one (should usually be youth who are wards of the state).
In addition to the above documentation we also need a monthly client summary report. We need the active/enrolled and inactive/discharged client summaries arranged by funding stream for each month. The client summary reports should have the following information: funding stream, youth name, youth DOB, ETO or TCM youth/family ID, caregiver name, caregiver contact, referral date, date of first CFT, care coordinator information, and care coordinator supervisor.
