Apply for MPH Diploma

MPH students are responsible for obtaining and completing all forms required for graduation and submitting them by the deadline dates to Teena Maultsby in the MPH Program Office.   

To apply for a diploma, login to SURFS (Student UseR Friendly System) for the form.  Your SURFS account was automatically created for you when you were admitted. To activate and access your account, go to the SURFS website at http://simsweb.umaryland.edu/ and follow the directions for accessing the system. 

SURFS Accounts are managed by the Office of Records and Registration.  If you have questions or problems with SURFS, contact them at 410 706-7480.
