Funding for C.H.A.M.P. is provided by the Preventive Health and Health Services Block Grant (award number 2007-B1-MD-PRVS) through the Centers for Disease Control and Prevention and the Maryland Department of Health and Mental Hygiene.

The University of MD, School of Medicine (Dept. of Family Medicine) serves as the administrative component.
