Leave of Absence Request Process for Fellows

Request should be submitted via scan attached to an email to Ms. Cindy Garnett, Academic Program Specialist in the Office of Academic Administration, at least one month in advance of the leave.

Attached to the email should be:

A written request from the fellow (email OK) including:

  • Start date of leave
  • End date of leave
  • Reason for leave

A scan of a signed letter from your chair/director addressed to Dean including (see template)

  • Full name of fellow
  • Fellow's supervisor
  • Start date of leave
  • End date of leave
  • Reason for leave
  • Indication of support

The Office of Academic Administration will seek Dean/Designee approval and UMB VPAA Office approval.  You will be notified if there are any problems or of approval/denial as soon as possible.
