Clinical Instructor/Fellow

**This is a faculty title given to Clinical Fellows who have been given a Maryland medical license before starting their fellowship and who will be seeing patients and will be billing for their services.

This action requires Dean's approval. Please submit the following documentation in a blue folder to our office:

• Letter from the Department Chair that includes all of the following details:
  • Full name of candidate
  • Academic degree held
  • Institution the fellow graduated from
  • Area of specialty
  • Nature of the fellowship
  • Nature of clinical participation
  • Name of Fellowship Director here at UMB
  • Start date
  • Length of the fellowship
   
• CV VERIFICATION FORM 
• CV (in school format) 
• 2 Letters of recommendation (Each letter should recommend the candidate for the fellow/faculty appointment, NOT for medical staff privileges.)
• Documentation verifying current licensure 
• Official copy of transcripts from institution granting the medical degree
• Faculty Appointment Information Sheet and SOM Salary Breakdown Sheet 
• Supplemental Data Verification Form
