The Division of Infectious Diseases is responsible for the inpatient HIV medical ward as well as consultation services at UMMS/VA, the Greenebaum Cancer Center, Shock Trauma, the transplant service and the virology consult service.

The HIV Medical Ward

The Med-ID service is one of the major teaching services of the School of Medicine. Most patients admitted to this service have advanced HIV infection and suffer from acute infectious complications (e.g. PCP, Cyptococcal meningitis, CNS toxoplasmosis, community-acquired pneumonia, skin and soft tissue infections, and bacterial endocarditis). In addition, the service includes patients without HIV infection who require acute hospitalization for management of complicated infectious diseases (such as endocarditis, meningitis, TB, and osteomyelitis). The service consists of two teams each which admits every other day. Team 1 consists of an ID attending with expertise in HIV medicine, a second or third year medical resident, and 2 interns. Team 2 consists of an ID attending with expertise in HIV medicine, an ID fellow who acts as a junior attending, and 2 interns. Both teams may have sub-interns and 1-2 third year medical students. The medical care provided on Med-ID includes a multidisciplinary team of nurses, social workers, physical therapists, substance abuse counselors, case managers and palliative care providers all working together to solve the problems commonly found among persons living with HIV infection.

The University Hosptial/VAMC Infectious Diseases Consult Services

Two service teams provide consultations throughout the University of Maryland Medical Center and the Baltimore Veterans Affairs Medical Centers on patients without HIV infection who have a wide variety of problems related to infectious diseases. One team, headed by an attending with expertise in the care of patients with infectious complications of neoplastic diseases, provides all of the consultations on patients in the Marlene and Stuart Greenebaum Cancer Center. In addition, both teams see patients throughout both hospitals. The University of Maryland Medical Center has one of the largest solid organ transplant services in the nation. Patients with solid organ transplants have an array of common and esoteric infections and non-infectious diseases that mimic infections. Both teams provide specialty consultative services to support this large population of immunocompromised patients. Each team consists of an ID attending, one or two ID fellows and a variable number of medical residents, fourth year medical students and PharMD students and interns.

The Union Memorial Hospital Infectious Diseases Consult Service

This consult service is located at Union Memorial Hospital approximately 10 minutes north of the University of Maryland Medical Center. Union Memorial Hospital is a 325 bed hospital with approximately 44,000 Emergency Room visits and 7500 admissions to the medicine service each year. This rotation provides general infectious diseases experience as well as infection control experience in a community hospital setting. The service consists of an ID attending, one ID fellow, and a variable number of medical residents and medical students.

The Virology Consult Service

This service staffed by an ID attending with expertise in HIV medicine and a second or third year HIV ID fellow provides consultations throughout the University of Maryland and Baltimore VA Medical Centers on patients with HIV infection not admitted to the Med-ID team. Similar to the Med-ID service, the patients seen on this service often have advanced HIV infection and suffer from acute infectious complications (e.g. PCP, cyptococcal meningitis, CNS toxoplasmosis, Community Acquired Pneumonia, skin and soft tissue infections, and bacterial endocarditis).

The Shock Trauma Consult Service

The ID Clinicians at the University of Maryland J. Adams Crowley Shock Trauma Center are an integral part of the team providing care for patients with severe trauma. Service on this team is one of the most popular electives for ID Fellows at the University of Maryland.

The Antimicrobial Management Program

Antibiotic management is becoming a major requirement of the infectious disease specialist at both community and teaching hospitals. We have established a strong antimicrobial management program at the University of Maryland. The program involves a coordinated approach with the infectious disease physicians, infection control and pharmacy. The Infectious Diseases Fellows are the first line in this program. They work closely with Dr. Forrest in the role of antibiotic restriction and Dr. Standiford with infection control. Our goals are to reduce inappropriate antibiotic usage in an effort to decrease antimicrobial resistant organisms, to educate physicians, and to contain costs. There are opportunities for fellows to pursue continued research in the area of antibiotic management.
