Reminders

  • For either appointment - all of the Chair’s signatures must be original; stamps are unacceptable.

  • Make sure all necessary information is included in the Chair’s letter.

  • If you have any questions please call our office.

To determine which action you should request please answer the following questions:

  1. Will this individual see patients and bill for their services?
If no, you should proceed with the request for a Clinical Fellow.
If yes, go to question 2.
  2. Will this individual have a Maryland license before they start their fellowship?
If no, you must proceed with the request for a Clinical Fellow.
**If the individual does not initially have a Maryland license they should be appointed as a Clinical Fellow. Once they receive their Maryland license you may submit a new request to make them a Clinical Instructor/Fellow.
If yes, you should proceed with the request for Clinical Instructor/Fellow.
